We are seeking a dedicated and caring person to join our team at Anglicare's Woolooware Shores Village, to provide a high level of service to our residents and village community. We serve our residents with care and compassion.
 
The role:
 
Reporting to the Village Manager, the role will suit someone with a Care Consultant, Case Management background or an Enrolled Nurse seeking a new opportunity to provide holistic care and support residents with identifying services and care needs.
 
Our team is seeking a new way to make a difference in aged care and we are passionate about serving seniors living residents. Our Health and Wellness Consultants play a key role in providing the very best care and support, to assist our valued residents to remain living independently at home for as long as possible.

Key responsibilities:
 
*In this role, you will provide support to the RN in guidance and comfort to our prospective new residents and families during their intake interview process.
 
*Your ability to easily build rapport with people and great listening skills, will ensure your success in this role and will equip you to work with our Seniors to identify support services to assist with independent living.
 
*The Health and Wellness Consultant will also work closely with our Anglicare at Home Community team, to implement plans to meet the social, service and health needs of our valued residents.
